It is important to take this medication exactly as prescribed by your doctor. Lowering high blood pressure helps prevent strokes, heart attacks, and kidney problems. If it is necessary to use MIDAMOR alone (see INDICATIONS), the starting dosage should be one 5 mg tablet daily. Amiloride is called a "water pill" (diuretic) and causes your body to get rid of extra salt and water while also preventing the kidneys from getting rid of too much potassium. If persistent hypokalemia is documented with 10 mg, the dose can be increased to 15 mg, then 20 mg, with careful monitoring of electrolytes. You should not use amiloride if you have kidney problems, if you are unable to urinate, or if you have high levels of potassium in your blood. This has been noted particularly in elderly patients, in diabetic patients, and in hospitalized patients with hepatic cirrhosis or cardiac edema who had known renal impairment, were seriously ill, or were receiving vigorous diuretic therapy.

The thing is you don't know which one is going to work well for you. Amiloride also helps to treat or prevent low blood potassium levels caused by the other diuretics. If the serum potassium level exceeds 6.5 mEq per liter, active measures should be taken to reduce it.

Amiloride is called a "water pill" (diuretic) and causes your body to get rid of extra salt and water while also preventing the kidneys from getting rid of too much potassium. Amiloride hydrochloride interferes with the mechanism involved in the exchange of sodium for potassium in the distal convoluted tubule and collecting duct of the nephron. It may also be used for other conditions as determined by your doctor. Amiloride hydrochloride is an antikaliuretic drug with mild natriuretic diuretic and antihypertensive activity.

Recommended dosage

Used as single agents, potassium sparing diuretics, including MIDAMOR, result in an increased risk of hyperkalemia (approximately 10% with amiloride). If you are not sure what to do after missing a dose, contact your doctor or pharmacist for advice. Do not give this medication to anyone else, even if they have the same symptoms as you do. It is also used to lower high blood pressure. The dosage may be increased to 10 mg per day, if necessary. Do not stop taking this medication without consulting your doctor. At approximately 5 or more times the expected maximum daily dose for humans, some toxicity was seen in adult rats and rabbits and a decrease in rat pup growth and survival occurred.

  • To help your doctor and pharmacist give you the best care, be sure to tell your doctor and pharmacist about all the products you use (including prescription drugs, nonprescription drugs, and herbal products) before starting treatment with this product. Maintenance doses may be lower than those required to initiate diuresis; therefore, reduction in the daily dosage should be attempted when the patient's weight is stabilized.
  • Many people using this medication do not have serious side effects. If your doctor has recommended a dose different from the ones listed here, do not change the way that you are taking the medication without consulting your doctor.
  • To help your doctor and pharmacist give you the best care, be sure to tell your doctor and pharmacist about all the products you use (including prescription drugs, nonprescription drugs, and herbal products) before starting treatment with this product. An increase in sodium and a decrease in potassium and hydrogen ion excretion are induced in the presence or absence of aldosterone, thereby suggesting a direct tubular action of the drug.
  • Avoid becoming overheated or dehydrated during exercise, in hot weather, or by not drinking enough fluids.

Side effects

It is usually used with other medicines. Many things can affect the dose of medication that a person needs, such as body weight, other medical conditions, and other medications. It has weak (compared with thiazides) diuretic and antihypertensive effects. If necessary, dosages of both drugs may be increased gradually until effective diuresis is obtained.

There are, however, no adequate and well-controlled studies in pregnant women. Talk to your pharmacist for more details. The principal use of amiloride hydrochloride is to conserve potassium in selected patients receiving kaliuretic-diuretic agents. Too much salt will cause your body to retain water and can make this medication less effective. Amiloride is called a "water pill" (diuretic) and causes your body to get rid of extra salt and water while also preventing the kidneys from getting rid of too much potassium. Impaired Renal Function Anuria, acute or chronic renal insufficiency, and evidence of diabetic nephropathy are contraindications to the use of MIDAMOR.

  • Amiloride is used with other "water pills"/diuretics (such as furosemide, thiazide diuretics like hydrochlorothiazide) to treat high blood pressure (hypertension), heart failure, or extra fluid in the body (edema). The dosage may be increased to 10 mg per day, if necessary.
  • Avoid becoming overheated or dehydrated during exercise, in hot weather, or by not drinking enough fluids. Tell your doctor immediately if you develop any symptoms of high potassium levels, including muscle weakness, slow/irregular heartbeat, numb/tingling skin.
  • It is used alone or in combination with a potassium-depleting diuretic (such as hydrochlorothiazide) to decrease edema (fluid retention) that occurs with congestive heart failure and disorders of the liver. Its effect on electrolyte excretion reaches a peak between 6 and 10 hours and lasts about 24 hours.

Dosage adjustment may be necessary. Because amiloride HCl is not metabolized by the liver, drug accumulation is not anticipated in patients with hepatic dysfunction, but accumulation can occur if the hepatorenal syndrom develops. The effects of some drugs can change if you take other drugs or herbal products at the same time. Midamor may increase the amount of urine or cause you to urinate more often when you first start taking it. This mechanism accounts in large part for the potassium sparing action of amiloride. In some cases, drinking too much liquid can be as unsafe as not drinking enough. Amiloride belongs to the class of medications called potassium-sparing diuretics (water pills). If any of these effects persist or worsen, tell your doctor or pharmacist promptly.

Precautions

Because amiloride HCl is not metabolized by the liver, drug accumulation is not anticipated in patients with hepatic dysfunction, but accumulation can occur if the hepatorenal syndrom develops. Potassium supplementation in the form of medication, potassium-containing salt substitutes or a potassium-rich diet should not be used with MIDAMOR except in severe and/or refractory cases of hypokalemia. Chloride excretion may remain unchanged or increase slowly with continued therapy. Patients with evidence of renal functional impairment (blood urea nitrogen [BUN] levels over 30 mg per 100 mL or serum creatinine levels over 1.5 mg per 100 mL) or diabetes mellitus should not receive the drug without careful, frequent and continuing monitoring of serum electrolytes, creatinine, and BUN levels.
